Welcome to mBIOTA Elemental
Your Elemental Diet Quick Start Guide
Everything you need to get started with your mBIOTA Elemental protocol is right here—from preparing your first shake to knowing what to expect along the way.
Follow the protocol recommended by your healthcare provider and use this guide for support along the way.
Step 1
Mix With Water
Mix one packet with 10 oz. of room-temperature water. Shake for 45–60 seconds until completely dissolved and serve cold, over ice, or blend with ice.
For best taste, consume within four hours.
Step 2
Sip Slowly
Elemental diets are designed to be absorbed rapidly by your digestive system. Sip each serving over at least 30 minutes. Drinking too quickly may cause bloating or lightheadedness.
Step 3
Stay Hydrated
Drink plenty of additional water throughout the day—not just the water used to prepare your shakes.
Tips for Success
A few simple strategies can make your elemental diet easier to follow. From finding your daily rhythm to staying hydrated and making each shake more enjoyable, these tips can help you feel prepared and stay on track throughout your program.
Tip 01.
Prepare for a Liquid Diet
Going fully liquid can be an adjustment. Mentally preparing before you begin can make it easier to stay committed once you get started.
Tip 02.
Remove Temptation
Keep tempting foods out of sight and consider asking those around you to avoid eating favorite foods in front of you, especially during the first few days.
Tip 03.
Plan Non-Food Activities
Meals are a big part of our routines. Plan social activities and ways to stay busy that don't revolve around food to help reduce temptation and keep yourself motivated.
Tip 04.
Mix Thoroughly
For the smoothest texture, make sure your shake is completely dissolved and free of clumps. A traditional blender works especially well; if using the shaker, shake vigorously for about 60 seconds.
Tip 05.
Drink It Cold
The powder dissolves best in room-temperature water, but the shakes taste best cold. Mix first, then serve over ice or chill before drinking.
Tip 06.
Make It a Slushie
Need a change of texture? Blend your shake with ice for a cold, slushy consistency and a little variety during an otherwise fully liquid diet.
Tip 07.
Stick to a Schedule
You may not always feel hungry, so don't rely on hunger alone. Most patients settle into a rhythm of a shake approximately every 2–4 hours to maintain consistent energy.
Tip 08.
Start Your Day Early
Even if you aren't normally a breakfast person, consider beginning your first shake shortly after waking to help spread your nutrition throughout the day.
Tip 09.
Try “Always Be Sipping”
If individual shakes leave you feeling an energy dip between servings, try sipping more slowly and consistently throughout the day, preparing your next shake as you finish the previous one.
Tip 10.
Adjust Water to Your Taste
You may adjust from 8–12 oz. of water per packet to customize the flavor. Use less water for a stronger taste or more water for a milder one, while keeping the shake mixed with water only.
Tip 11.
Give Yourself Time to Adjust
The first few days are generally the most challenging. Headaches, fatigue, cravings, and digestive changes may occur as your body adjusts to a fully liquid diet. Be patient and listen to your body.
Tip 12.
Keep Track of How You Feel
Consider logging your daily experience—including bowel movements, GI discomfort, fatigue, symptoms, and notes. This can also give you useful information to share with your healthcare provider.
During The Elemental Diet
What to Expect
Everyone’s experience with an elemental diet is different, but knowing what may happen along the way can make the process feel more manageable. Here’s a general look at what you may experience throughout a typical 14-day protocol.
Days 1–4
Getting Adjusted
The first few days—especially Days 1–2—are typically the biggest adjustment. You may experience nausea, diarrhea, or other digestive changes as your body adapts. Plan accordingly, stay close to a bathroom if possible, drink plenty of water, and give yourself time to settle into your new routine.
Days 5–7
Finding Your Rhythm
By now, you may be settling into a routine and noticing fewer cravings. Some people experience fatigue, brain fog, or digestive changes during this stage, and it’s possible to feel worse before you begin to feel better. Everyone’s experience and timing can be different.
Days 8–11
A Turning Point
Around Day 8, many people begin to notice that the protocol feels easier and symptoms may start to improve. Bloating, digestive discomfort, energy, or brain fog may begin to shift, although everyone's response is different. If you aren't feeling better yet, that doesn't necessarily mean the diet hasn't been effective; some people don't experience the full effect until after completing the protocol.
Days 12–14
The Final Stretch
As your GI tract rests, bowel movements may become less frequent or stop altogether because very little waste is being produced. This is generally expected. Stay consistent with your protocol, rinse your mouth between shakes to help with flavor fatigue, and begin preparing for your transition back to food.
Returning To Solid Foods
Depending on your individual protocol, your healthcare provider may recommend continuing the elemental diet beyond 14 days or beginning your transition back to food. When it’s time to reintroduce food, follow the 7-Day Low Fermentation Eating (LFE) Transition Plan to gradually add foods back while giving your digestive system time to adjust.

Frequently Asked Questions
Mixing & Drinking
Mix one packet with 8–12 oz. of water, depending on your taste preference. Use less water for a stronger flavor or more water for a milder flavor. To maintain the elemental nature of the shake, mix with water only.
Add one packet to water in a 16 oz. or larger container. Room-temperature water helps the powder dissolve more easily. Cover tightly and shake for 45–60 seconds, or until completely dissolved. You can also use a traditional blender.
It really depends on your personal preference - some people sip continuously all day (make a new shake every time they finish one but are just sipping slowly throughout the day). Others scheduled “meals” every 2-4 hours. It’s really about what finding what works for you, but it's important to stay ahead of any sensations of hunger. We recommend taking at least 30 minutes to drink each one - drinking faster or chugging could result in bloating or discomfort (much like with any liquid).
During Your Elemental Diet
Always consult with your health care provider before beginning any elemental diet as your nutritional needs may vary, however when being used as sole source nutrition, no additional foods or liquids (aside from water) are consumed in order to allow your system to rest and reset. mBIOTA Elemental is designed to deliver all your necessary vitamins, minerals and nutrients.
Plain, unsweetened and unflavored seltzer or sparkling water may also be consumed, but please be mindful of any bloating or discomfort that could result from the carbonation.
Depending on the condition you are managing, mBIOTA Elemental can also be used as supplemental nutrition, or used to replace only a few meals each day. It's best to consult with your health care provider about the appropriate use of the elemental diet for your particular needs and diagnoses.
Initially, in the first few days, your body may push out residual waste that has been stored in your colon. Around days 6-7, some people experience yellow or green diarrhea - this is a good sign! It indicates that the intestines are naturally shedding their lining, which happens every five days but is usually unnoticed in a regular diet. This is part of the body's natural GI reset process and an indication that the diet is working as intended.
Mild diarrhea is common on this diet. During our 2023 Clinical Trial, patients experienced less frequent but looser bowel movements. Their bowel movements returned to a normal consistency - and even improved after the elemental diet. The percentage of normal bowel movements increased from 56% before the elemental diet to 71% 2 weeks after completing the elemental diet.
Fewer bowel movements while on the elemental diet is also normal. The diet itself does not cause constipation, but can slow down bowel function as the GI tract rests and little to no waste byproduct is created. This is normal and generally no cause for concern. Miralax offers an over-the-counter remedy for constipation that is not contraindicated while on the elemental diet. As always, listen to your body, and if feeling any overwhelming discomfort, please discontinue the diet and consult your health care provider.
Generally, we advise that you stick to just the shakes and water when following the exclusive protocol. One cup of plain black coffee or tea, no creamers or sweeteners (including non-nutritive sweeteners like monk fruit and stevia) is allowed. As your body acclimates to the diet, it will be more susceptible to caffeine and the amount you may have tolerated on Day 1 will not be as well tolerated on Day 5 for example. As always, please consult with your health care professional.
It is important to remember that every patient's experience using mBIOTA Elemental can be different, and it is often difficult to determine whether any side effects are the result of the elemental diet, bacterial die-off, or the original GI condition itself. In general, some patients report feelings of fatigue or brain fog, as well as an increase in diarrhea (usually green or yellow in color). These symptoms can be attributed to bacterial die-off and are generally an indication that the product is working as intended.
The timing of these experiences varies from person to person. Symptoms can appear as early as days 3-5, or as late as days 6-8, and in many cases, it is possible to feel worse before you begin to feel better on the diet. This is considered entirely normal.
However, please follow up with your health care provider if you feel any discomfort.
Medications & Supplements
We do have several patients who do the elemental diet following a course of antibiotics. While, in general, there is no particular amount of time you need to wait before beginning the elemental diet, we do recommend double-checking with your health care provider based on your individual needs and diagnosis. We also generally do not recommend using antibiotics and an elemental diet at the same time, unless otherwise instructed by your health care provider.
We recommend that you discuss your specific medications and situation with your health care provider to ensure safety and efficacy during your use of the elemental diet. While most medications are generally compatible with the mBIOTA Elemental diet, please be sure to discuss this with your health care provider prior to starting the diet, particularly if any of your medications are required to be taken with food.
Returning to Solid Food
We've compiled a list of helpful tips on how to return to solid foods in a way that should help you sustain balance of your gut microbiome and work to reduce the chances of a future flare-up.
Please note, if using the elemental diet as your sole source of nutrition, your stomach size will shrink over the course of the two-week period - it is recommended to eat smaller portions initially to avoid any feelings of discomfort.
For ongoing maintenance, we recommend the Low Fermentation Eating (LFE) diet created by Dr. Mark Pimentel and Dr. Ali Rezaie.
